I understand that my membership, and that of my travelling companions (if any) to the VIP Association is subject to the receipt by the VIP Association of the full subscription amount as indicated in my application for membership.

I understand that by submitting this application I am authorizing the Visitors International Protection (VIP) Association and its agents to release any provided medical information of the VIP Association Member or Members to any service provider deemed by the VIP Association to require such information in order to provide services as subscribed to by the VIP Association Member.

I understand that authorizing the disclosure of this health information is voluntar.

I understand that I am not required to sign this agreement/authorization.

I understand that I have the right to revoke this agreement/authorization at anytime.

I understand that if I revoke this agreement/authorization that I must do so in writing and present my revocation to the VIP Association Member Services Department.

I understand that revocation will not apply to information that has already been released in response to this authorization.

I verify that the information provided in this agreement/authorization is accurate in all respects.

I understand that it is the sole responsibility of the VIP Association Member or Members to renew and update this agreement/authorization as required.

I understand that it is the sole responsibility of the Member and/or Members to inform the VIP Association of any changes in medical conditions, medications, allergies, or any personal wishes as expressed in this agreement/authorization.

I understand that in the event of my providing false information, or commiting any fraudulent act relating to the use of the VIP Association services, my membership to the Association will be summarily cancelled and I will be excluded from all the related services and priviledges.

I understand that the VIP Association will make any and all attempts necessary to establish contact with the VIP Association Member and/or Members designated Contacts in the event that a verified emergency incident has occurred.

I understand that VIP Association will not be held liable for failure to establish contact with any of the above listed Contacts utilizing the Contact information provided in this agreement/authorization.

I understand that I have the right to inspect and/or receive copies of the information contained in this agreement/authorization to be used or disclosed.

I understand that a photocopy of this agreement/authorization shall be valid as the original.

I understand information contained in this agreement/authorization can be added or changed at anytime with the written consent of the VIP Association Member or Members by completing an VIP Association Change of Information official form.

The VIP Association Change of Information form can be downloaded from the VIP Association website, www.VIPA911.com, or obtained by request by calling +27 (0)80 435 7911.

I understand that the VIP Association will make every effort within reason to verify the existence of an actual emergency situation involving the VIP Association Member or Members in this agreement/authorization before attempting to make contact with any of the designated emergency contacts listed in this agreement/authorization.

Should the VIP Association become unable to verify an emergency situation, VIPA911 contact centre will not pursue any attempts to establish contact with any emergency contact.

In the event that the order of emergency contacts to be notified of an event is not clearly defined by myself, I understand that the default order in which emergency contacts will be contacted is as follows: Spouse > Mother > Father > Sister > Brother > Son > Daughter > Aunt > Uncle > Niece > Nephew > Cousin > In-Law > Other _______________________

I understand that the VIP Association will not be held liable for failure of emergency first responders to utilize the services offered by VIPA911 SPOC. It is neither the responsibility nor the priority of Emergency First Responders, including but not limited to; police, fire, EMR, EMS, or hospitals, to utilize the services offered by the VIP Association Disclaimer: The VIP Association provides a single emergency point of contact (VIPA911 - SPOC) for visitors to South Africa.

VIPA911 refers emergency and related assistance requests to third party service providers. VIPA911 does not render emergency and related services and the cost of such services are not included in the VIPA911 subscription fee.

Any service provided by a third party service provider to the VIP Association member shall be subject to the service provider’s standard terms and conditions, and shall be billed by the service provider directly to the VIP Association member and shall be for the account of the VIP Association member.

Where VIP Association has negotiated preferential rates with service providers, such preferential rates shall be passed on to members.

The VIP Association makes no warranties, implied or otherwise as to the level of service provided by third party service providers, but will use its best efforts to contract only with reputable service providers and to continually monitor the level of service provided.

The services of VIPA911 should never take precedence over the rendering of emergency medical attention on behalf of the VIP Association Member or Members/Victim.

The services offered through the VIP Association shou ld only be utilized once the proper authorities deem the emergency situation has become stable.

VIP Association will not be liable for any damages sustained by any member, of any nature whatsoever, arising from the negligence or any act of omission by VIP Association’s employees, agents and-or service providers, nor for the Association’s inability to perform any of its obligations due to Acts of God, Governmental Intervention, strikes or any other event whatsover outside the reasonable control of the VIP Association.
